• Our clinic does not participate with or bill any insurance plans, other than Medicare Part B.

• Payment of all Fee-for-Service Fees and any applicable co-insurance and deductible for Medicare Part B is due at the time of service.

• Memberships can be canceled at any time, and Membership Fees will be subject to a prorated refund based on remaining whole months in the applicable annual or semi-annual Membership term; Monthly Plan fees and Registration/Re-Enrollment Fees are non-refundable.
