  • A one-time $199 registration/re-enrollment fee applies to monthly plans (charged at your first visit).

  • Monthly fees are due on the 1st of each month.

  • Services outside of membership benefits are billed at our fee-for-service rates. 

  • Family Discount: If one family member joins, a spouse or adult child (18+) receives 10% off the same plan.
